When applying for a Nationally Coordinated Criminal History Check, you are required to provide the details of all names under which you are or have previously been known. This includes your current legal name, any alias under which you are currently or have previously been known, and all previous names (maiden or married names).
If you have been married more than once, you will need to provide the details of all your previous married names. If you are known by a shortened version of your legal name, you will need to add this name as a alias.
If your identity documents display a combination of your legal name and a previous or alias name, you will be required to provide a Change of Name document alongside your identity documentation. For more information, see our article on Change of Name documentation.
