When applying for a police check, you are required to provide the details of all names under which you are or have previously been known. This includes your current legal name, any alias under which you are currently or have previously been known, and all previous names (maiden or married names).
If you have been married more than once, you will need to provide the details of all your previous married names. If you are known by a shortened version of your legal name, you will need to add this name as a alias.
